Body Mass Index reporting

December 1, 2017 As part of a required school health examination, a student is weighed and his/her height is measured. These numbers are used to figure out the student’s body mass index or ‘BMI’. The BMI helps the doctor or nurse know if the student’s weight is in a healthy range or is too high or too low. Recent changes to the New York State Education Law require that BMI and weight status group be included as part of the student’s school health examination. Our school district has been selected to take part in a survey by the New York State Department of Health for the 2017-2018 school year and we will be reporting to New York State Department of Health information about our students’ weight status groups.

If you do not wish to have your child’s weight status group information included as part of the 2017-2018 Health Department’s survey, please print and sign your name below and return this form by December 20, 2017.


All students entering 7th, 8th and 12th grades must have the meningococcal vaccine.
Without it, they can’t start school.

Please be sure your child is vaccinated with the meningococcal vaccine - if he or she is entering grades 7, 8 and 12 - before school begins this September.

Fall 2017 sports physicals due to Health Office by Aug. 7

Physicals for FALL sports are due in the Health Office by August 7th. There will be no physicals offered at school before that time. Please make arrangements with your own doctor and have that information faxed to (518) 661-7666.

Sports Physicals

May 6, 2015 The N.Y.S. Education Law requires that all students entering the 7th and 10th grade and those planning on participating in a sport, have a physical examination completed and on file in the Health Office. Read the complete letter here.

Health departments provide advice on preventing the spread of respiratory illness known as Enterovirus D68 (EV-D68)

September 17, 2014 As you may be aware from recent media coverage, several states in the Midwest reported clusters of a respiratory illness known as Enterovirus D68 (EV-D68) affecting children and others over the last few weeks; more recently, cases of EV-D68 have been confirmed in New York. The Mayfield Central School District is providing information from the Centers for Disease Control and Prevention (CDC) and the New York State Department of Health (DOH) to help parents and community members become better informed about the illness and to help prevent the spread of the illness.  Read more
